All health intake and consent forms must be submitted at least 48 hours prior to your first visit. Go ahead and do them now to avoid forgetting — I love early!

Free street parking is available all around the building:
Park Ave (Front of building, closest to main entry) — No time limits, follow street signs
26th Street (North side of building) — 2 hour limit, no parking from 4 - 6pm
Columbus Ave (Back of building) — 2 hour limits, follow street signs
There’s also a paid ramp marked Green Ramp on 26th if you prefer.

The building is mostly residential living and secure — all doors are locked.
Preferred Entrance
Use the front doors facing Park Avenue on the west side of the building.
Enter the glass doors under the metal portico
Dial128 (if you forget, I’m listed under L - Lange) on the call box and I’ll buzz you in
Go through the lobby, up 3 steps, and take the elevator down to the G Level
If you prefer stairs go past the elevator, look left and you’ll see an exit/stair sign to take
Go to G level
Follow the sign posted outside the elevator bank toward Unit B4
Keep going through an extra door almost all the way down the hall
B4 will be on your left — enter the big doors marked Spaces at Life Healing Life
Come on in — make yourself comfortable in the waiting room and I’ll be with you shortly!
Second Entrance
You can enter the north side door on 26th Street under the black fabric awning.
I’ll need to personally let you in, so please park then text me at 612-423-9986
Sit tight until I write back which may not be until your visit time if I’m with another patient

Virtual Video
Just before your appointment, I’ll send you a link to sign in to a FaceTime video call. It’s private, secure, encrypted, and works with all types of devices. If you use Windows or Android devices you must have a Chrome browser installed.
Voice Only
I’ll simply call you at the number listed on your forms at the time of your visit.

If you are on my standby list you will be able to schedule any available visits in the upcoming 2 weeks. This helps me continue to prioritize my more established patients while also giving you the opportunity to get started.
You must submit your new patient online forms in advance of your first visit. Then, if something fits your schedule you can grab it without having to scramble to still complete forms — I need them at least 48 hours ahead of a first visit. Once more consistent availability is in my schedule I will reach out and provide you with my established patient scheduling password.

Payments can be made in person or via invoice. All invoices are sent by email the following Monday offering secure payment.
I do not accept insurance. Learn more about submitting visits to your own insurance for reimbursement.
